At Louetta Family Medicine, we seek to provide compassionate, patient-centered care and treatment from early life to adolescence and throughout all phases of adulthood. Our mission is to create a warm and welcoming medical environment that recognizes the importance of primary care through education, clinical expertise, good communication and professionalism. Our board certified providers are knowledgeable, experienced, and capable of offering a wide variety of well care services, medical diagnoses, and treatment options.

    We currently offer the following services in our practice . . .

  1. Pediatric services including children’s well-child exams, immunizations, and sick visits for children ages 2 and up.
  2. Wellness care, annual checkups, and immunizations for men and women of all ages.
  3. Worker’s compensation evaluations and treatments for those injured on the job.
  4. Women’s Healthcare including family planning, contraception counseling, gynecology care, PAP smears, pelvic exams, breast exams, and treatment of the symptoms of menopause.
  5. Complete Physical Examinations for school, sports, camp, and pre-employment, including DOT and executive physicals.
  6. Evaluation and Management of Acute Conditions such as allergies and sinusitis, bronchitis, ear infections, sore throat, headache, sleep disorders, chest pain, abdominal pain, urinary tract infections, hemorrhoids, back pain, joint pain, tendonitis, and bursitis.
  7. Evaluation and Management of Chronic Illnesses such as arthritis, asthma, chronic lung disease, diabetes, high blood pressure, heart disease, high cholesterol, and osteoporosis.
  8. Evaluation and Management of Skin Conditions such as acne, cysts, infections, moles, rashes, and sun damage, including cancerous and pre-cancerous skin conditions.
  9. Physician monitored weight loss plans custom-tailored to your lifestyle, needs and goals.

We make referrals

If a patient is in need of a medical or surgical specialist with additional expertise in a specific area, we will gladly recommend a specialist physician and provide any necessary referrals.

What is a physical/checkup/well exam?

An office visit for a physical/checkup/well exam involves a medical examination of a healthy person with no symptoms. During the visit, we look for hidden disease and consult with the patient about healthy living habits. The purpose of this visit is to keep you healthy through disease prevention and early detection of any health problems. A well visit is covered by most insurance. This visit does not include prescription refills, evaluations of new symptoms, or management of chronic problems.

How long does it normally take to schedule an appointment at your practice?

We make every attempt to schedule appointments quickly and at the convenience of the patient. Same day care for sick visits can normally be arranged but cannot be guaranteed. Please call us for up to date information on scheduling.

How much is my co-pay and when is it due?

Your co-payment amount is determined by your specific insurance carrier and policy. The entire amount of your co-pay is due on appointment day when services are rendered. We recommend that you contact your insurance company’s customer service department for all of your insurance related questions. They are in the very best position to assist you.

What is a new symptom visit?

If you’re experiencing a new health problem, issue or discomfort, an appointment to investigate your new symptom(s) will be necessary. Some symptoms are rather simple to treat and can be resolved with one office visit. Other issues may involve complex or multiple symptoms and may require multiple office visits. New symptom visits do not include medical refills for other problems or any preventive services. Nearly all insurance plans covers office visits for new symptoms, including Medicare and Medicaid.

How long will I have to wait before being seen?

We know that your time is valuable. In our efforts to minimize wait times, we schedule patients reasonably and run a well-organized and efficient practice. Despite our best intentions, it is inevitable that emergencies can arise in a medical practice and this can always offset our patient schedule. If this situation arises on the day of your appointment, please know that we do respect your time and are doing everything we can to get you seen in a timely manner.

Do you accept my insurance?

We accept most major insurance plans. Please call our office for more information.

What are follow-up or chronic care visits?

Your provider may request that you come back to our practice at a later date for a follow-up visit so that your previous symptoms and condition may be re-evaluated. This type of visit can also be used to review chronic problems, adjust medications, refill prescriptions etc. This type of visit does not include any well care services and is covered by almost all insurance policies, including Medicare and Medicaid.

How do doctors decide what to focus on during my appointment?

From reflexes to heartbeats and beyond, medical professionals are extensively trained to examine patients for numerous physical signs and conditions. When you come in for an appointment, your provider will choose what to focus on by relying on your medical history and considering any symptoms or problems that you may be experiencing.

Do you file my insurance?

For your convenience, we will gladly file your insurance claim on your behalf. It is important to note that your insurance policy is an agreement between you and your insurance provider. Therefore, we cannot become involved in disputes between you and your insurance company regarding coverage and/or policy benefits.

What is an administrative physical?

An office visit for an administrative physical requires that your doctor conduct a specific examination of your physical condition and complete a detailed form. These types of physicals are typically required for camp, sports, school, work or insurance purposes. Administrative physicals are often times not covered by insurance, though this varies by policy. Administrative physicals do not include medical refills or evaluations of new symptoms or concerns.

How do I communicate with your practice?

Good patient communication is very important to us. During normal business hours, you may always call our front desk for any questions or assistance. Our patients also have the option of connecting to our secure patient portal at anytime for billing information, medical records, prescription refills, or appointment requests. Lastly, if you have an urgent need after regular office hours, you may call us and our answering service will connect you with our on-call physician.

Can I combine multiple services into one visit?

While it is sometimes possible to schedule a follow-up visit on the same day as an administrative physical, for example, this is only done for a matter of patient convenience and will likely result in separate charges for each service provided. Therefore, we may collect two co-pays at the time of your visit and submit two charges, depending on your insurance benefits.
