When it comes to selecting a family practice, we realize that you have a choice and a very important decision to make. As experienced primary care providers with a passion and commitment to high quality healthcare, we hope you’ll consider experiencing the difference that our practice has to offer.
Insurance
We accept coverage from most major insurance carriers. These include but are not limited to Aetna, BCBS, Cigna, MHMD Memorial Hermann, and United Healthcare. For your convenience, we will glady file your insurance claim on your behalf. Please call our office so that our staff may help you determine your patient benefits as estimated by your insurance carrier.
Don't Forget!
It is very important that you bring your insurance card with you to every appointment, as this enables us to file the insurance claim on your behalf!
A Few Extra Reminders:
- Unless you’ve already printed and completed all new patient paperwork, please arrive 15 minutes early to allow enough time to do so.
- If you have an HMO insurance policy, be sure to change your Primary Care Provider (PCP) to the name of one of our providers. If it is not changed prior to the appointment, you may be asked to reschedule or will be responsible for all services provided during your appointment.
- It is important that you know your co-payment amount and come prepared to pay this amount. It is due on the day of the appointment.

In exchange for our dedication, we ask that our patients become familiar with certain responsibilities that will allow us to continue to operate an efficient practice for all of those we serve.
Your Rights
As a patient at Louetta Family Medicine, you have the right to receive quality healthcare and be treated respectfully by our providers and staff. You also have the right to expect that all communications and records pertaining to your healthcare will be treated as confidential and will not be disclosed to any person without your permission, unless otherwise required by law. Finally, you also have the right to receive assistance from your insurance company’s customer service department for any and all questions related to your policy and coverage.
Patient Responsibilities:
- Please make every attempt to follow your physician’s instructions. Never hesitate to call us if you have questions.
- Please be on time for your appointment. We know that situations arise, but if you are more than 15 minutes late we may have to ask you to reschedule.
- We ask that you please inform us of changes related to your address, phone number, or insurance information.
- Please make it a habit to always carry your insurance card and bring it with you to every appointment.
- Please be aware of your insurance benefits and the amount of your co-pay, which is due at the time of your appointment.
- If a referral is needed to a specialist, please inform our office of this at least 48 hours before your appointment.
- Allow 48-72 hours for all prescription refills. Please call the pharmacy and request a refill on your medication before calling our office.

Get registered in our system
Before becoming a new patient, we will need to have all of your basic information on file, including health history. Once you are registered in our system and have submitted insurance information, we will be able to quickly help you in a time of need.

Once you become a patient, you'll have access to our medical staff during regular office hours, our on-call physician after hours, and our secure patient portal 24/7. The portal is helpful for requesting appointments, prescription refills, and getting medical records.
